2026-08-17 Adderall Shipment Returned to Sender

What happened

Sherlette Smith renewed both Adderall scripts after the 2026-07-20 appointment and Optum Home Delivery filled the order, but the delivery failed. The package required a signature; the July 31 attempt was missed (“Notice Left — No Authorized Recipient Available”), no second attempt was made, and the redelivery I scheduled for Aug 12 never happened. USPS held the package the required number of days, then marked it “Unclaimed/Being Returned to Sender” on Aug 14. Note: I remembered the carrier as UPS, but tracking is USPS (9297490350467327121685).

Called OptumRx (1-855-814-3512) on 2026-08-15. The pharmacy rep said they will call the prescriber today (Mon 2026-08-17) — either to get authorization to resend or to try to recover the package from the carrier — and will expedite given I am out of medication. They asked me to also contact my provider directly.

2026-08-17: Call to Mount Sinai

Couldn’t find a direct number in the Mount Sinai app (messaging-focused), so found the office number via a Zocdoc listing for Sherlette Smith and called. Spoke to a rep and described the situation — some mutual confusion, but she pulled up my chart. Sherlette isn’t in until Wednesday (2026-08-19); the rep will try to route it to another provider who can assist. She said updates typically appear in my chart in the Mount Sinai app — check there if no callback.

What OptumRx actually needs

Two tracks, depending on where the package is:

  1. Recover in transit — while the package is still with USPS on its way back to Lenexa, KS, the original dispensing remains valid; Optum can intercept and redirect it with no new prescription. This is the time-sensitive option the rep described: it only works before the package is received back at the pharmacy.
  2. Replacement fill — once received back, returned Schedule II meds can’t be restocked or re-dispensed (they’re destroyed). A replacement is then a new dispensing event, and since C-II has no refills, it needs a fresh e-script (EPCS) or documented prescriber authorization. This is why they’re calling the prescriber in parallel as the fallback.

Package left Union City 2026-08-14; return transit to KS is roughly 5–7 days, so the intercept window closes around 2026-08-19/21.

Replacement fill route, not intercept. Optum confirmed the return on 2026-08-24 (“The mail carrier returned your order — call us before placing your new order”), and a replacement order shipped the next morning. This time it went UPS (not USPS), which turned out to matter: UPS My Choice allowed a redirect to an Access Point after the missed attempt, so no second return-to-sender cycle.

Gap in the email trail: how the replacement got authorized between 08-17 and 08-25 (covering provider vs. Sherlette Smith back on 08-19) isn’t in Gmail — likely handled via the Mount Sinai app or by phone. Total time without medication: roughly 2026-07-20 fill runout → 2026-08-29 pickup, minus whatever buffer was left from the June fill.

Lesson: for signature-required C-II shipments, redirect to a UPS Access Point / hold-for-pickup on the ship-notification day, before the first attempt. Waiting for a missed attempt burned three days here and cost the whole order last time with USPS.
